IRA Rollover

Question: if you rolling over your fidelity 401k from one company to another fidelity 401k what documents do you need?

Hi i liquidated my bank of America 401k and am putting into another 401k with the another company that uses fidelity. now I’ve filled out most of the incoming rollover contribution application. now since they use the same investment company do i still to get a determination letter and distribution statement or would i just sign it and send it back with the check from my old 401k and on part of the application ask for a certification signature but i think that’s for rollovers from a conduit ira… could anyone provide any help?

Answer: Call Fidelity and ask for a rollover specialist. They were very helpful to me in getting all the correct paperwork to rollover my 403(b).
